To make your care at LifeForce Chiropractic more enjoyable, we have prepared some of our documents ONLINE for new patients. Having these documents filled out prior to your initial arrival will help minimize your wait time on your first visit. Please click on the appropriate link below for your category, to access the documents you will need for your first visit. If you are not sure which category to pick, please contact our front desk staff at (321) 409- 0209 and they will help guide you to the correct forms to speed up your initial visit.
Once you have completed your forms online, you can select the appropriate links below, to print and sign any of the additional required documents, and bring them with you on your first visit. If you do not have access to a printer you can still sign these in our facility as well.

If necessary all FORMS may be filled out in the office. However, this will take a greater amount of time and your appointment will have to be scheduled accordingly. Please let our front desk staff know your circumstance so we can maximize your time with the Doctor.
